Your Patient will qualify for care if:

  • They have a medical reason to receive home health services
  • They have difficulty moving or leaving home unassisted.
  • They are compliant with their doctor's visit schedule.
  • They have a plan of care.
  • They are in need of Skilled Nursing Care, Physical/Occupational/Speech Therapy or other Skilled Medical Services.
  • For Medicare patients,  there must have been a face to face encounter between the patient’s physician and the patient 90 days prior to being admitted into the home health program or 30 days after the start of care at  which time the physician must certify patient as being qualified to receive home health services. Certification documentation is required. (Ask Agency for documentation guidelines).
